The applicant will need to complete the Application for special migration to Australia (PDF, 567KB) form, if the applicant will be bring dependent family members then the details of children or other dependent family members aged 18 years or over (PDF, 238KB) form will need to be completed for each dependent relative aged over 18, including those not included in the visa application.
